Crawley Community Partnership Team

CPT Crawley Neighbourhood Network works in close partnership with the Crawley Community Partnership Team (CPT).

The CPT is a joint team including staff from West Sussex County Council, West Sussex Primary Care Trust, Department for Work and Pensions, Age Concern West Sussex, 4Sight and the Alzheimer's Society.

How can they help?

The CPT will work with an older person to advise or actively put in place a range of measures which will assist that person’s health, quality of life and ability to live independently. The CPT normally works with people who do not receive services from specialist health care teams or social care services.
